Halaxy Account Setup for Solo Clinicians

Please have the following ready before completing the form. Having everything on hand will make the process much faster:

• Business name and ABN
• Practice name, address/es, and phone number
• Admin or billing email address
• Bank account details (account name, BSB, account number)
• Medicare Provider Number/s
• Professional registration number (AHPRA, AASW, PACFA, or equivalent)
• Fee schedule — initial session, regular session, concession, and cancellation fees
• Practice logo file (PNG or SVG, high resolution)
• Digital signature — sign on white paper, photograph or scan, and save as PNG
• DVA, WorkCover, or NDIS provider details (if applicable)

We'll ask about your preferences in the form, but it helps to think about these beforehand:

• Intake process — do you prefer to send the intake form before booking, or after?
• Payment method — Halaxy card, bank transfer, or both?
• Credit card on file — mandatory, optional, or not required?
• Online bookings — would you like clients to be able to book themselves?
• AI note-taking — if you use an AI scribe, we'll need to add a separate consent clause to your intake form
